REGISTRATION
To enroll at Swainsboro Primary School:
- All students must have current immunization on the appropriate State Form 3231
- Eye, Ear and Dental Form 3300
- Social Security Card or waiver
- Certified birth certificate to verify date of birth
- Proof of residency
Also required for admission:
- Completed Registration Form containing working phone numbers for use in case of emergency
- Persons to contact in case of emergency
- Persons eligible to pick up the child
- Current mailing address
- 911 address
- Home and work phone numbers
EMERGENCY CONTACT INFORMATION
- It is critical for the school to be able to contact parents at any time students are at school.
- The school must have the parents’ current address and home, cellular, and business telephone numbers.
- Emergency contact persons and their telephone numbers are needed in the event a parent cannot be reached.
- This information is required at the time of registration and whenever a change occurs with the parent’s address, telephone, or emergency contact information.
- If circumstances occur resulting in changes being made regarding emergency contact persons (i.e., custody disputes), it is the responsibility of parents to notify school authorities.
- THIS IS FOR THE SAFETY AND WELL-BEING OF YOUR CHILD.
CUSTODY
If custody of a minor child changes after enrollment, documents should be provided to the school as soon as possible after the change. Students cannot be withdrawn without this information.
It is a crime for any person to make or attempt to make a change of custody of a minor child by removing the child from school without permission of the person who enrolled the child in school, even if the person attempting to remove the child has a court order granting that person custody.
Court orders that specifically authorize or direct the release of custody by the school system will be followed. The school system will not interject itself in a custody or visitation dispute between parents. The school system will give to non-custodial parents, upon request, all information required under the Family Educational Rights and Privacy Act and the laws of the State of Georgia unless there is a valid court order directing the school system not to divulge such information. If such an order exists, a certified copy must be delivered to the principal’s office.
